The statutory and private health insurance companies form the basis of the health insurance system in Germany. In the case of public health insurance companies, most of the benefits are covered by a specific catalog of benefits, the framework of which is defined in the Fifth Social Code (SGB V) and whose details are decided by the Federal Joint Committee (G-BA). Employees or employees subject to social security contributions are compulsorily insured up to a certain income threshold in the statutory health insurance (GKV). Self-employed persons and employees with an income above the mandatory insurance limit (€ 59,400.00 in 2018) can, however, choose whether to switch to private health insurance (PKV).

In the case of private health insurance, the assumption of costs for the benefits is defined individually upon conclusion of the contract. For example, there are basically considerable differences in the range of services and contribution rates for private health insurance. For example, the latter often increase significantly with increasing age as they are based on the costs incurred. In contrast, the amount of membership fees in statutory health insurance is based on the income of the insured and not on the costs incurred or the state of health of the insured. However, there are also differences between the individual statutory health insurances despite the alignment with the common benefit catalog.
